Part 1
Copies of the following items must be turned in with your background paperwork. Failure to provide these items will result in the delay of your application and may cause your removal from the selection process.

Part 2
The following forms and documents must be downloaded and completed. The forms may be printed and filled in by hand or applicants can choose to complete them using typewriter or computer etc. The applicant is responsible for turning in the printed, completed forms.

5. Waivers and Miscellaneous Documents Instructions for form
6. Background Investigation Booklet Instructions for form
Part 3
Applicants who proceed beyond the initial application and background investigation processes will also be required to provide: (The background investigator will advise you when these additional documents will be required)

DO NOT BREAK SEAL ON CERTIFIED DOCUMENTS - OPENED DOCUMENTS WILL NOT BE ACCEPTED
