Copies of the following items must be turned in with your background paperwork. Failure to provide these items will result in the delay of your application and may cause your removal from the selection process.

2. Social Security Card
3. High School Diploma or GED
4. College / University Diploma
5. Marriage License:
Must have license from all marriages
6. Divorce Paperwork:
Must have paperwork from all divorces
7. Bankruptcy Paperwork
8. DD214 (Long Form Only) (Former Military Personnel Only)
9. Credit Report:
No Older than 90 days. Visit

Part 2
The following forms and documents must be downloaded and completed. The forms may be printed and filled in by hand or applicants can choose to complete them using typewriter or computer etc. The applicant is responsible for turning in the printed, completed forms.

Part 3
Applicants who proceed beyond the initial application and background investigation processes will also be required to provide: (The background investigator will advise you when these additional documents will be required)

1. Certified Copy of your Birth Certificate
2. Certified Copy of College / University Transcripts if applicable
3. Certified Copy of High School Transcripts to Verify graduation if applicable
